Contact Forms Problem

It seems that all the forms on my website, when tested, produce the following error:

Unknown column 'forms' in 'field list'

The contact form is emailed to me - so it is submitted. But it gives the user the impression of an error. Also, it appears that some custom changes made to the forms are no longer working. For example, I paid to have the form display that it was submitted successfully after it sent. I also paid to have the results of the form displayed in the user profile on the backend in the NOTES section.

Any ideas here?

This problem is occuring again! Any ideas?

It was an interesting one - seemed to work fine - then it would only break when I filled it in incorrectly at first - then corrected the required fields and sent it in again - the issue was that a field was missing from the users table - which has now been resolved.
