Programming will have to provide an estimate for building that form - as for a design estimate, it depends. You could simply link to a page with the form yourself, or you could get a designer to create a div for the body content, or a space in the side-nav, and fit it in there. That could take 30min-2 hrs, depending on what you want.
I like the seoncd choice but need to have a better price estimate.

I propose that you should have a programmer create the form functionality (30 min programming, Aaron says), and we'd get a designer to place it in a styled box (30 min), similar to the "featured areas callouts", but less detailed. This would be a snippet that you could put in any body's content.
I think it wouldn't be practical to have it in a side-nav, since we want to have a decent amount of space for the "feedback" comment field.
