For my own websites...
First, create an "Icon" file.
Name the icon file, "favicon."
When creating the Icon use 16x16 as your Icon size.
Then upload the Icon file to your root directory.
Insert this code on your page:
<link rel="shortcut icon" href="http://www.yourdomain.com/favicon.ico">
